What Happens If You Stop Gender-Affirming Hormone Therapy?

People may pause or stop gender-affirming hormone therapy for many personal or medical reasons. After stopping, some hormone-related changes may gradually reverse if the body resumes its own sex hormone production, while changes such as voice deepening or breast development may remain. Because effects vary, discuss any treatment changes with a gender specialist.

Can You Stop Gender-Affirming Hormone Therapy?

No two gender transition journeys are identical. Some people take gender affirming hormones for life. Others decide to pause treatment, change their hormone regimen or stop HRT (MTF or FTM) altogether.

So the short answer is yes. But what happens if you suddenly stop gender HRT?

This is a valid question to ask. However, there is no fixed answer. Some effects of gender HRT can gradually start to reverse when your body’s own sex hormone production resumes, while others are likely to remain. Fertility may recover in some individuals but can’t be guaranteed.

Abruptly stopping gender-affirming hormone therapy can cause a rapid drop in sex hormone levels, triggering severe fatigue, hot flushes and intense mood fluctuations. Tapering off under medical supervision can help ease this hormonal transition, minimising physical discomfort and emotional distress.

If you’re considering gender affirming hormone therapy (GAHT) or have already started, understanding what happens when transgender people stop taking hormones and discussing it with a gender specialist can help you make an informed gender healthcare decision.

Why Might Someone Decide to Pause or Stop HRT?

Many reasons can lead someone to decide to pause or stop transgender hormones. And it does not necessarily mean that they regret transitioning or that their gender identity has changed.

Some common reasons for people to stop transgender hormones include:

  • Achieving some of the desired physical changes

  • Changes in transition goals, circumstances or priorities

  • Exploring a different hormone regimen

  • Experiencing unwanted side effects

  • Health issues prompting someone to reconsider treatment

  • Trying to preserve or restore fertility

  • Preparing for a medical procedure

In some non-binary people, GAHT may have been used to achieve certain desired changes rather than as a long-term hormone treatment.

Whatever the reason, changing your mind about ongoing gender HRT should be discussed openly with your clinician. And if you do stop transgender hormones, it’s more important to work out what stopping means medically for you.

What Happens to Your Hormone Levels When You Stop Gender HRT?

What happens to your hormone levels depends on whether your ovaries or testes are still present and functioning.

If You Have Your Ovaries or Testes

If your ovaries or testes are present and functioning, stopping transgender hormones and hormone-suppressing medication may allow your body’s endogenous hormone production to increase again.

This means that if you stop MTF HRT and still have functioning testes, your body’s testosterone production may recover. As testosterone becomes the predominant sex hormone, some of the feminising effects that depended on testosterone suppression may also change.

However, these changes take time and don’t happen overnight.

If You Have Your Ovaries or Testes Removed

If both testes have been removed, your body may not resume normal testicular testosterone production. Similarly, removing both ovaries may mean ovarian hormone production can’t restart. In these circumstances, stopping gender affirming hormone therapy suddenly may leave sex hormone levels very low and should be discussed with a clinician. They may also want to talk to you about the risk of osteoporosis, hot flushes and chronic fatigue.

This is why you should not stop hormone treatment without clinical advice after gonad-removing surgery.

Which Changes From Gender HRT May Be Reversible?

Depending on how long you’ve been on gender HRT, your hormone levels and related factors, reversible or partly reversible changes include:

Body fat distribution: Stopping transgender hormones can cause your body fat distribution to shift slowly.

Muscle mass and strength: Testosterone promotes greater muscle mass while feminising HRT can reduce it. Stopping gender HRT can change your muscle mass and strength.

Skin changes: Stopping testosterone may reduce oiliness and clear acne, while stopping oestrogen HRT may make skin oilier again.

Menstruation: If ovaries and uterus are retained, periods may return after stopping testosterone.

Sexual function and libido: Sexual function, erections and libido may change after stopping transgender hormones.

Which Changes From Gender HRT May Be Permanent?

Some physical changes caused by GAHT may be permanent or may not fully reverse, even after treatment stops.

Changes That May Remain After Masculinising HRT

  • Deeper voice

  • Facial and body hair growth

  • Scalp hair loss

  • Clitoral growth

The amount of change that stays even after stopping testosterone HRT varies.

Changes That May Remain After Feminising HRT

  • Breast development

Once the underlying glandular tissue has developed, stopping oestrogen does not make the tissue disappear.

Fertility does not fit neatly into either category, because fertility may recover after stopping transgender hormones, but this is not guaranteed. This uncertainty is why we recommend discussing fertility preservation options before starting HRT.

What Happens If You Want to Restart HRT Later?

“Can you stop taking HRT and then start again?” is a common question to ask. Some people pause gender HRT temporarily and later decide to restart. But restarting hormones doesn't mean the older prescriptions or doses will still be appropriate. Many aspects of your health can change after being away from gender HRT.

Before restarting gender HRT, your clinician may need to review:

  • How long you’ve been off HRT

  • Your current medical history

  • New health concerns

  • Your current transition goals

  • Relevant blood test results

  • Any side effects experienced before

  • Any medications you have started or stopped

  • Fertility considerations

Your treatment plan can be reconsidered based on these factors.

Why You Should Speak to Your Gender Specialist Before Stopping HRT

There is a key difference between choosing to stop gender HRT and stopping HRT at any time without understanding the medical consequences.

Your gender specialist can determine whether treatment should be stopped, reduced, modified or paused and explain what you’re likely to experience.

This matters especially if:

  • Your testes or ovaries have been removed

  • You take hormone blockers alongside HRT

  • You want to preserve fertility or become pregnant

  • You are planning surgery

  • You are taking other medicines that may interact with your hormone treatment

  • You are stopping due to side effects or health issues

  • You are unsure if you want to stop completely

The right medical guidance from UK gender specialists can help make any change to your gender HRT regimen safer, so you are not left dealing with the medical consequences alone.
