Adults in the UK can access private gender-affirming hormone therapy without a GP referral, but treatment should still involve appropriate clinical assessment, prescribing and ongoing monitoring. Eden New Life provides a clinician-led private pathway that does not depend on NHS shared care, while clients can continue involving their GP in their wider healthcare where appropriate.

Gender Affirming Hormone Therapy Without a GP

If you’re currently considering starting gender-affirming hormone therapy (GAHT), you may have assumed the first step is to speak to your GP for a referral. While this is a valid route, it’s not the only way to start gender HRT.

Adults in the UK can approach private gender care providers such as Eden New Life directly and start gender HRT without a GP referral. We manage your treatment privately through our network of independent UK gender specialists.

But starting on gender hormones without a GP referral does not mean you don’t need clinical supervision before and during treatment. Gender affirming hormones are prescription medicines with significant and sometimes permanent effects. Appropriate assessment, prescribing, blood tests and ongoing monitoring are integral to safe and effective gender-affirming hormone care.

So what does starting gender HRT without a GP referral actually include?

Do You Need a GP Referral to Start Gender Hormone Therapy in the UK?

As an adult, you don’t necessarily need your GP to refer you before you can access gender-affirming hormones from a private provider. It depends on the path you choose.

In England, adults can currently be referred to NHS Adult Gender Services by a GP or another NHS healthcare professional through the Gender National Referral Support Service. After specialist assessment, adults can then be offered treatments including gender hormone therapy, where clinically appropriate.

However, UK private gender healthcare providers work differently. Private gender care providers such as Eden New Life accept people directly. This means you can contact the provider yourself without first seeking permission or referral from your GP. Access to starting HRT for FTM or MTF, or any particular gender hormone treatment or prescription, is still subject to individual clinical evaluation.

What Are Your Options for Starting HRT Without a GP?

If you don’t want to depend on your GP to start gender hormones, these are the available options.

Self-Refer to a Private Gender Healthcare Service

Instead of asking your GP for a referral, you can register with a private healthcare provider yourself. The provider can guide you through their assessment process, costs and how prescriptions and monitoring are handled.

Receive HRT Through a Private Prescription

If gender hormone treatment is considered clinically appropriate, a licensed UK prescriber may issue a private prescription. You will have to pay privately for the medication, as well as the relevant private consultations, blood tests and other services not provided by the NHS.

At Eden New Life, we send prescriptions from our independent specialists to our pharmacy partner electronically. This pharmacy sends the client a payment link before next-day delivery of the medication, tracked by courier.

Stay on NHS Waiting List While Using Private Care

Many people still want their GP involved in their general healthcare or prefer to remain within an NHS gender-service pathway while accessing private treatment.

However, it’s important not to assume that starting private gender HRT means your GP will automatically take over your gender prescription. NHS and private care operate under different arrangements, and GP involvement and prescribing need to be agreed in advance.

Note: Eden New Life does not offer shared care arrangements with the NHS.

Avoid Self-Medicating with Hormones

Starting HRT without a GP referral does not mean DIY HRT. Buying hormones yourself or using medication not specifically prescribed for you removes important safeguards.

The NHS strongly recommends that people don’t use treatment that has not been prescribed for them. If you don’t want to wait for your GP or the NHS gender services to start treatment, consider choosing a clinically supervised and monitored private gender healthcare service.

How to Start HRT Through a Private Gender Service

Starting MTF or FTM with a private provider involves more than just securing a prescription. Ideally, your MTF or FTM journey should follow a structured process, starting with an assessment of your current health parameters and circumstances.

At Eden, our subscription plans for gender affirming hormone therapy start with an intake session, careful diagnosis and blood tests before the prescribing stage. Our specialists provide follow-ups every 12 weeks to monitor a client’s well-being and progress.

You don’t need to arrive at Eden with a separate formal diagnosis report from another clinician or psychologist. A specialist diagnostic assessment by a qualified, experienced clinician is already part of your subscription plan before you start gender HRT with us. This is one advantage of subscribing to Eden New Life.

What Role Can Your GP Still Play in Your Care?

Your GP remains part of your general healthcare, and you can keep them informed about the medication you’re taking. You can ask your GP whether they are willing to arrange blood tests, but they are not required to provide monitoring on behalf of a private gender service provider.

At Eden, we encourage our clients to keep their GP in the loop. We can provide a free template letter to do so. If your GP wishes to contact us, we welcome it, but we don’t ever disclose patient information without your prior written consent.

However, this does not mean Eden offers shared care with GPs. Our current model covers treatment, monitoring and specialist oversight through our own network of independent clinicians, rather than relying on an NHS GP to accept a shared care agreement.

Can Your GP Refuse to Prescribe Gender-Affirming Hormones?

Yes, because GPs must take clinical responsibility for the prescriptions they sign. According to the General Medical Council (GMC), when doctors share responsibility for a patient’s care, they must be competent to take on their part of clinical responsibility and have appropriate arrangements in place.

Moreover, entering a shared care agreement with a private gender healthcare provider is a voluntary choice for a GP. If a GP is uncertain about taking responsibility, the GMC allows them to make appropriate arrangements for the patient’s continuing care. The Royal College of General Practitioners recognises that many GPs will not feel confident or competent starting gender hormone prescriptions without specialist input.

This means that an assessment or a recommendation from a private clinician does not automatically require an NHS GP to take over prescribing.

With Eden, your gender HRT pathway does not depend on a shared care agreement. You can ask your GP for blood tests, but if your GP declines, Eden can offer access to private at-home testing services.

How to Start Gender HRT Without a GP Referral Through Eden New Life

Transgender, non-binary and intersex adults in England, Scotland, Wales and Northern Ireland can explore our private online pathway to gender-affirming hormone care.

  • You can come to Eden without a GP referral or a formal diagnosis report. Explore our plans and choose a paid subscription that suits you.

  • Start with an intake session where our specialists will discuss your medical history, gender experiences and HRT goals. Next, they’ll do a diagnosis session where they conduct a clinical assessment. After that, they will likely request that you take a comprehensive blood test before any medication can be considered.

  • Our specialist prescribers only provide prescriptions after an expert clinician determines that treatment is clinically safe and appropriate for you.

  • Any prescriptions are passed electronically to Eden’s pharmacy partner, who will contact you to process payment and arrange delivery. Once you pay them for the medication securely online, the pharmacy dispenses the medication and delivers it to you via next-day courier service.

  • Your care does not end here. It continues through regular reviews and monitoring.

All Eden sessions are fully online and included in the subscription. Medication and blood tests are paid for separately unless your plan includes blood tests, and prices vary depending on the person.

The important point here is that you don’t need a GP referral to start exploring gender HRT with Eden. The care you receive from us is structured, clinician-led and non-judgmental throughout your journey.
